Аннотация

Цель исследования – анализ осложнений одномыщелкового эндопротезирования коленного сустава и их причин. С ноября 2011 г. по июнь 2015 г. было выполнено 141 частичное замещение коленного сустава 137 пациентам. Преобладали женщины – 124 (90,5%). Возраст больных варьировал от 27 до 84 лет, средний возраст составил 65 лет. Проведено 137 операций эндопротезирования внутреннего компартмента. Осложнения возникли у 14 (9,9%) пациентов (14 суставов): интраоперационный перелом мыщелка бедра – у 2, нестабильность полиэтиленового голенного компонента – у 5, прогрессирование артроза противоположного отдела сустава – у 2, интерпонент в суставной щели – у 3, боль неустановленной причины – у 2. Артроскопическое удаление фрагмента цементной мантии, резекция псевдомениска и гипертрофированной си- новиальной оболочки купировали болевой синдром у 3. В остальных наблюдениях произведено ревизионное тоталь- ное замещение коленного сустава с хорошим результатом. Отмечены некоторые особенности конверсионных опера- ций при нестабильности голенного компонента.To analyze complications after unicondylar endoprosthesing of the knee joint and their causes. 137 patients had 141 partial replacement of their knee joints from November 2011 till June 2015. These patients were mostly women (90.5%). Patients' age varied from 27 till 84 years (average 65) . There were 137 surgeries for endoprosthesting of inner compartment. There were complications in 9.9% (14 patients) : intraoperative fracture of the hip condyle -2; instability of polyethylene crural component -5; progressing arthrosis in the opposite part of the joint - 2; interpotent in the joint gap; pain of unclear origin - 2. The arthroscopic removal of cement mantle, resection of pseudo meniscus and hypertrophic synovial membrane promoted the controlling of pain syndrome in 3 patients. In other cases a revisional total replacement of the knee joint was done with good results. Some peculiarities in conversional surgeries in case of the instable crural component were described as well.
