Аннотация
Кровопотеря и потребность в переливании крови является актуальной проблемой в травматологии и ортопедии, в частности, при эндопротезировании тазобедренного сустава. Транексамовая кислота - современный фармакологический препарат, способный влиять на периоперационную кровопотерю.
Цель исследования. Оценка эффективности и безопасности внутривенного использования траксексамовой кислоты в периоперационном периоде при эндопротезировании тазобедренного сустава.
Материалы и методы. в одноцентровое проспективное рандомизированное исследование было включено 108 пациентов, которым выполнялось первичное эндопротезирование тазобедренного сустава по поводу коксартроза. Пациенты были рандомизированы в 2 клинические группы: 1 группа (n=54) пациенты, которым выполняли инфузию транексамовой кислоты 1 грамм внутривенно за 20 минут до кожного разреза, 2 группа (n=54) пациенты, которые не получали инфузию транексамовой кислоты. В периоперационном периоде оценивали уровень гемоглобина (до операции, после операции, через 24 – 48 часов), интраоперационную кровопотерю, необходимость в проведении гемотрансфузии, возникновение побочных эффектов: тромбозов вен нижних конечностей и тромбоэмболических осложнений.
Результаты. Уровень гемоглобина после операции и через 24 часа был выше у пациентов 1-й группы, чем у пациентов 2-ой группы. Интраоперационная кровопотеря была достоверно ниже у пациентов 1-й группы, в сравнении со 2-ой группой. Осуществление гемотрансфузии потребовалось 6 пациентам из 2-ой группы. Достоверных различий по частоте тромбозов и тромбоэмболических осложнений между группами отмечено не было.
Выводы. Периоперационное внутривенное введение транексамовой кислоты позволяет снизить интраоперационную кровопотерю, сохранить более высокий уровень гемоглобина и снизить частоту гемотрансфузии. Использование транексамовой кислоты не увеличивает частоту развития тромбозов и тромбоэмболических осложнений.
