Аннотация
Цель. сравнительный анализ ближайших и отдаленных результатов применения стентов с лекарственным покрытием и изолированной баллонной ангиопластики.
Материалы и методы. Обобщен опыт 398 эндоваскулярных вмешательств (на 323 конечностях у 297 пациентов) по поводу критической ишемии нижних конечностей в период с 2015 по 2020 гг.. Проведен ретроспективный анализ с применением метода подбора по индексу соответствия (PSM).
Результаты. В группе вмешательств с применением стентов в течение года «большие» ампутации были выполнены 10% больных, а регресса ишемии и сохранения конечности достоверно удалось добиться у 69% пациентов. Выявленная летальность составила 6% от общей численности группы. В группе больных с изолированной баллонной ангиопластикой достоверно регресса ишемии удалось добиться у 50% пациентов, «большие» ампутации выполнены в 14% случаев, показатель летальности составил 8%.
Заключение. В группе больных с применением стентов с лекарственным покрытием наблюдалась тенденция к более высокой частоте сохранения конечностей в течение 12 месяцев наблюдения. Первичное применение стента с лекарственным покрытием в зонах стенотического поражения в ряде случаев может позволить избежать нежелательных диссекций, а имплантация стентов в области устьевых поражений, по-видимому, позволяет улучшить прогноз в случае необходимости повторных вмешательств. Результаты. показывают, что имплантация стента может оставаться предпочтительной стратегией в особых обстоятельствах, но для формирования окончательного вывода необходимы долгосрочные рандомизированные исследования с большим количеством изучаемых групп и унифицированными методологиями.
