Аннотация

Удаление новообразований лобных пазух и решетчатого лабиринта является непростой задачей, даже для опытных хирургов. Существует множество видов доступа при патологии фронтоэтмоидальной области, включающие наружные и эндоскопические подходы. В статье представлены и систематизированы современные хирургические подходы, применяемые в отделении оториноларгинологии ФГБУ «ЦКБ с поликлиникой» УД Президента РФ при лечении доброкачественных новообразований, включающих остеомы, мукоцеле и инвертированные папилломы, с локализацией в области лобной пазухи, лобного кармана и крыши решетчатого лабиринта. Хирургические вмешательства включали наружный доступ и эндоскопический эндоназальный подход с применением стандартной и расширенной фронтотомии III типа (Draf III). Бикоронарный остеопластический лоскут применяли при первичном вмешательстве в тех случаях, когда новообразование занимало большую часть объема пазухи или располагалось латерально, что исключало возможность применения исключительно эндоназального подхода. Радикальную операцию на лобной пазухе с разрезом по надбровной дуге применяли у ранее оперированных пациентов с имеющимся косметическим дефектом. Эндоскопический эндонозальный доступ применяли самостоятельно либо в сочетании с наружным подходом. Исключительно эндоназальный подход применяли в тех случаях, когда новообразование располагалось в решетчатом лабиринте или области лобного кармана, и в тех случаях, когда новообразование не являлось причиной сужения носолобного соустья или не требовало полного удаления. Учитывая высокий риск внутриоперационных осложнений и послеоперационного стеноза носолобного соустья, предложены методы, позволяющие выполнять операции качественно и способствующие формированию стойкого носолобного сообщения. Методы включили высверливание дна лобных пазух с объединением в единую полость и пластику сообщения с использованием свободных лоскутов слизистой оболочки.To remove neoplasms in the frontal sinuses and in the ethmoid sinus is a challenge, even for experienced surgeons. There are many types of surgical approaches for the treatment of frontoethmoidal pathologies including external and endoscopic ones. The article presents systematized modern surgical approaches which are implemented in the ENT department of Central Clinical Hospital with an outpatient unit subordinate to the Affair Management Department of the President of Russian Federation for treating benign tumors, including osteomas, mucocele and inverted papillomas located in the frontal sinus, in the frontal pocket and in the roof of ethmoid labyrinth. The surgical procedures included an external approach and endoscopic endonasal approach with standard and extended frontotomy of type III (Draf III). A bicoronary osteoplastic flap was used in primary interventions if the tumor occupied a larger part of the sinus or was located laterally, what excluded endonasal approach. A radical surgery in the frontal sinus with a cut on the brow arc was performed in previously operated patients having cosmetic defects. Endonasal approach was used only when neoplasms were located in the ethmoid labyrinth or in the area of frontal pocket, and when the neoplasm did not narrow the nasalfrontal anastomosis or did not require a radical removal. Given the high risk of intraoperative complications and postoperative stenosis in the nasal- frontal anastomosis, the developed techniques allow to perform such surgeries successfully with a stable nasal- frontal anastomosis. The techniques included drilling of the frontal sinuses floor with the formation of united cavity as well as anastomisis plasty with free flaps from the mucous.
