Аннотация

Цель исследования – изучение эффективности и безопасности применения метотрексата и УФВ 311 нм фототе-рапии у пациентов с развившимся «эффектом ускользания» на фоне терапии блокаторами фактора некроза опухо-ли α. Представлены результаты применения метотрексата в дозе 15–20 мг в неделю и УФВ 311нм фототерапии (курс25–30 процедур) у 12 пациентов, получающих терапию инфликсимабом на фоне вторичной неэффективности. Мо-ниторинг пациентов соответствовал стандартам ведения больных на биологической терапии. Эффективность оценива-лась по данным динамики индексов PASI, BSA, ДИКЖ. В результате исследования было показано, что в случае разви-тия резистентности при применении препаратов биологической терапии (блокаторы ФНО-α) целесообразно назна-чение метотрексата в дозе 20 мг в неделю на протяжении до 6 мес. У пациентов с недостаточным эффектом метотрек-сата (снижение PASI менее чем на 50%) целесообразным является включение в терапевтический комплекс УФВ 311 нмфототерапии курсом 25–30 процедур. Комбинированное применение биологической терапии и метотрексата (+УФВ311 нм фототерапии) в лечении больных вульгарным псориазом с «эффектом ускользания» способствует выраженно-му регрессу клинических симптомов и позволяет контролировать процесс, что подтверждается динамикой индексовPASI, BSA и ДИКЖ.Ключевые слова: псориаз, системная терапия, биологическая терапия, блокаторы фактора некроза опухоли α,метотрексат, УФВ 311 нм фототерапия, PASI, BSA, ДИКЖ.To study the efficacy and safety of Methotrexate and UVB 311 nm phototherapy in psoriatic patients with developed"escape effect" during their treatment with blockers of tumor necrosis factor α. The authors prescribed Methotrexate in thedosage of 15-20 mg / week and UVB 311nm phototherapy (one course 25–30 sessions) to 12 patients who had been prescribedInfliximab therapy earlier with no effect. Patients' monitoring complied with the standards on managing patients whoreceive biological therapy. The efficacy was assessed by the dynamics of indexes PASI, BSA, DLQI. This study has shownthat in case of resistance to preparations of biological therapy (blockers of TNF-α) it is reasonable to prescribe Methotrexatein the dosage 20 mg / week up to 6 months. Patients with the insufficient effect of Methotrexate (PASI decrease by lessthan 50%) should be added with 311nm UVB phototherapy for 25–30 sessions. The combination of biologic therapy andMethotrexate (+ UVB phototherapy 311nm) utilized in the treatment of patients with psoriasis vulgaris and "escape effect"contributes to pronounced regression of clinical symptoms and allows to control the process analyzing dynamics of indexesPASI, DLQI and BSA.Key words: psoriasis, systemic therapy, biological therapy, blockers of tumor necrosis factor-α, Methotrexate, UVB311nm phototherapy, PASI, BSA, DLQI.
